REQUIRED APPLICATIONS/FORMS

Volunteer Application. Anyone who serves in nursery through youth group must fill out this application for a police check/screening.

Youth Leader Driving Declaration. This form must be filled out by any volunteer who plans on driving students for any particular outing.

Youth Registration/Permission Form. Each student in our youth group is required to have an annual registration/permission form on file with current contact numbers and emergency information. This will also allow participation in all Overflow events without requiring an additional permission form per event.
